245D Services are community-based supports for individuals who need ongoing assistance to live safely and independently outside of institutional settings. At Alpha Care Group LLC, these services follow Minnesota’s Home and Community-Based Services (HCBS) 245D standards, which guide how care is assessed, planned, and delivered.
In Minneapolis and across Hennepin, Ramsey, Washington, Anoka, Scott, and Dakota counties, services typically begin with a county assessment to determine eligibility and level of need. From there, an individualized support plan is developed, and services are provided in the home or community based on the person’s daily routines. Support is adjusted over time, with staff coordinating with case managers and documenting care to ensure it remains consistent with state requirements and the individual’s needs.

Individual Home Supports (IHS)

In Home Support Without Training
Who it’s for:
Children or adults who need basic help with daily routines and getting out into the community.
What the service includes:
- Help with daily personal care (like bathing, dressing, eating, and using the toilet)
- Support with household routines (like cleaning, meals, and laundry)
- Supervision and reminders to stay on track
- Help getting out into the community (like going to the store or a walk)
- Emotional support and companionship
Individual Community Living Support (ICLS)
Who this service supports:
Children, adults, or seniors with mental health needs, physical disabilities, or developmental disabilities.
What the service includes:
- Provide support with ADLs and IADLs, including personal care, meal prep, housekeeping, budgeting, and medication reminders.
- Assist with community access, appointments, social activities, and maintaining engagement outside the home.
- Follow each person’s Individualized Service Plan (ISP) while promoting independence, safety, and daily living skills.
In Home Support With Training
Who it’s for:
Adults who need help learning or practicing life skills to become more independent.
What the service includes:
- All the support listed in IHS Without Training
- Teaching and practicing life skills (like cooking, cleaning, budgeting, or using public transportation)
- Setting small goals and tracking progress
- Encouraging independence in daily tasks
Homemaker/Cleaning
Who this service supports:
Clients who need help maintaining a clean, organized, and safe home environment.
What the service includes:
- Light and heavy cleaning
- Household organization and management
- Support with shopping, cooking, laundry, and transportation
In Home Support With Family Training
Who it’s for:
Children or adults who live with family members who also need support in caring for them.

- All the support listed in IHS Without and With Training
- Teaching family members how to support the client at home
- Helping the family build safe and consistent routines
- Sharing tools to manage stress, behavior, and safety
Respite Care
