In Minneapolis and across Hennepin, Ramsey, Washington, Anoka, Scott, and Dakota counties, these services are commonly accessed through Medical Assistance programs. The process typically begins with a county assessment that determines eligibility and the level of support needed. From there, a care plan is developed, and a caregiver is assigned based on the individual’s preferences, schedule, and type of assistance required.
Care is delivered in the client’s home, where routines are already familiar. Caregivers follow the approved plan closely, document visits as required, and communicate with families or case managers when changes are noticed. Personal Care and Activities of Daily Living Assistance
PCA/CFSS caregivers provide hands-on support with Activities of Daily Living (ADLs), based on the individual’s care plan and assessed level of need. These are essential tasks that many individuals may find difficult to complete independently due to age, disability, or health conditions.
Services are delivered with attention to safety, privacy, and routine. Caregivers are trained to assist without taking over completely, encouraging as much independence as possible while reducing risk.
ADL support may include:
- Dressing and undressing
- Grooming and personal hygiene
- Bathing and skin care
- Safe transfers (bed, chair, wheelchair)
